Medical Release will only need to be filled out once a year unless any information needs to be updated.
Code of Conduct form will need to be filled once for the year.
Parental Release form will need to be filled once for the year.

We know that no one likes to fill out paperwork, but we do this to ensure the safety for our students and in case of any emergencies.  Turn in forms to either Jason Turner or Jason Pennington.
